Registration Rules

*** USER NOTICE ***

During filling your online application form, carefully upload your information details like name, surname, date of birth, father’s name/husband name. During receiving of your original certificate it will be same as per your uploaded information. Council will not take any responsibility for any mistake.

Kindly carry your two copies of passport size photos with all uploaded documents both original and attested photocopies along with your printed application form and paid receipt copies during visit to the office of West Bengal Pharmacy Council to receive your original registration certificate.

This is to notify to all Applicants those who have passed out from other State Pharmacy Institutions to select the respective State from scroll bar. Otherwise the online application will be automatically cancelled.

New Registration

For Degree Holders

1. All Marksheets of 8 Semesters of Bachelor of Pharmacy.

2. Training Certificate (One month For Industrial Training or 150 hours For Hospital training).

3. Age Proof Certificate (Class X Admit Card/ Birth Certificate)

4. I.D. Proof: Voter I.D. Card/ Aadhaar Card/ Passport

5. Character Certificates (In Original)

6. 10+2 Marksheet (Scan Copy of Original)

7. Attested Copy of Degree Certificate or provisional Degree certificate.

8. Three Recent Passport Size Photographs (5 X 4 Cms)

9. An affidavit (as per specimen copy in Website download section in case of applicant who has passed Degree in Pharmacy examination from an Institution within West Bengal and from other states for undertaking mentioning the authenticity of all documents submitted during New Registration.

10. Proof of Employment of the candidate:

(a) Employer's Certificate mentioning period of service, designation, head quarters, etc. from relevant Firm/Company/Institution mentioning Drug License No’s, if any ; or
(b) In case of self employed persons(License): A self declaration on the letter pad of the relevant Firm/Company etc. mentioning Drug License No’s. if any.

11. Affidavit ( First Class Judicial Magistrate) as per specimen in Website Download Section in case of applicant who has been registered as pharmacist in other state Council.

For Diploma Holders

1. All Marksheets Of Diploma in Pharmacy (Part-I & Part-II)

2. Practical Training Certificate of 500 Hours (Original Copy)

3. Age Proof Certificate (Class X Admit Card/Birth Certificate)

4. I.D. Proof: Voter I.D. Card/ Aadhaar Card/ Passport

5. Character Certificates (In Original)

6. 10+2 Marksheet (Scan Copy of Original)

7. Three Recent Passport Size Photographs (5 X 4 Cms).

8. An affidavit (as per specimen copy in Website download section in case of applicant who has passed Diploma in Pharmacy examination from an Institution within West Bengal and from other states for undertaking mentioning the authenticity of all documents submitted during New Registration.

9. Proof of Employment of the candidate:

(a) Employer's Certificate mentioning period of service, designation, head quarters, etc. from relevant Firm/Company/Institution mentioning Drug License No’s, if any ; or
(b) In case of self employed persons(License): A self declaration on the letter pad of the relevant Firm/Company etc. mentioning Drug License No’s. if any.

For Pharm D Holders

1. All Marksheets of Pharm D Course.

2. Internship Certificate of one year

3. Age Proof Certificate (Class X Admit Card/Birth Certificate)

4. I.D. Proof: Voter I.D. Card/ Aadhaar Card/ Passport

5. Character Certificates (Original Scan Copy)

6. 10+2 Marksheet (Original Scan Copy)

7. Three Recent Passport Size Photographs (5 X 4 Cms).

8. Pharm D Certificate.

9. An affidavit (as per specimen copy in Website download section in case of applicant who has passed Pharm D examination from an Institution within West Bengal or from other states for undertaking mentioning the authenticity of all documents submitted during New Registration.

11. Affidavit ( First Class Judicial Magistrate) as per specimen in Website Download Section in case of applicant who has been registered as pharmacist in other state Council.

Reciprocal Registration

For Degree Holders

1. All Marksheets of 8 Semesters of Bachelor of Pharmacy.

9. Submission of requisite fees of Rs. 1500 (Rupees Fifteen Hundred).
Submit Original Registration Pharmacy Certificate issued by other State Council of India directly to the office of the WBPC within ten days from the date of receiving of SMS and confirmation Mail from WBPC of submission of requisite fees.

10. Affidavit ( First Class Judicial Magistrate) as per specimen in Website Download Section in case of applicant who has been registered as pharmacist in other state Council.

11. Proof of Employment of the candidate:

(a) Employer's Certificate mentioning period of service, designation, head quarters, etc. from relevant Firm/Company/Institution mentioning Drugs License No’s, if any ; or
(b) In case of self employed persons, A self declaration on iletter pad of the relevant Firm/Company etc. mentioning Drug License Nos. if any
